HIV和SIV都属于逆转录病毒，牛津大学的研究表明，逆转录病毒可能已存在了5亿年，逆转录病毒或许起源于古代海洋，并随着动物宿主从海洋来到陆地[7]。美国亚利桑那大学和图兰大学的研究人员根据对比奥科岛SIV的遗传分析认为，SIV至少存在了32000-75000年时间。在奥比岛与非洲大陆分离的1万年前，SIV已经存在于整个非洲大陆，对数千万年前猿猴的DNA样本进行检测验证了SIV的远古起源[8]。
如今能够确定的是，HIV经历了至少13次的SIV跨物种传播至人类[9]。如图 2所示，黑猩猩的SIVcpz是三种不同SIV菌株重组事件的结果。除了一种未知的SIV菌株外，重组还涉及到今天感染猴的SIVgsn/mon/mus分支的前体，也可能是今天来自红帽白眉猴的SIVrcm的前体。黑猩猩病毒随后传播给大猩猩和人类，分别产生SIVgor和HIV-1M组和N组。HIV-1组O和P组是SIVgor两次人畜共患传播事件的结果，而SIVsmm感染白眉猴的情况至少有9次传播给人类，导致HIV-2组A到I的出现。
